Unit stock number, Full Name (on credit card), Credit Card info (includes card number, expire date and the CCV number on Visa/Mastercard/Discover-located on rear of card after account number). The ship to address must match the credit card billing address (this is for your protection). Also have an email address for warranty transfers as well as valid phone number needed for UPS shipping.
